What Age Should You Start Botox? A Doctor's Honest Advice
The Rise of Preventative Botox
"Baby Botox" and "preventative Botox" have become buzzwords on social media, with influencers in their early 20s sharing their treatment experiences. But as a doctor, I believe it's important to separate marketing trends from medical evidence.
The truth is: there's no single "right age" to start Botox. The decision should be based on your individual skin, your concerns, and — most importantly — honest medical advice from a qualified practitioner.
Understanding How Wrinkles Form
To understand when Botox might be beneficial, it helps to know how wrinkles develop:
Dynamic Wrinkles (Movement Lines)
These appear when you make facial expressions — frowning, raising your eyebrows, squinting. In your 20s, these lines disappear when your face is at rest. Over time, repeated muscle movement causes these lines to become etched into the skin permanently.
Static Wrinkles (Resting Lines)
These are visible even when your face is completely relaxed. They develop from a combination of:
- Repeated dynamic wrinkle formation
- Loss of collagen and elastin (natural ageing)
- Sun damage (photoageing)
- Genetics
Botox addresses dynamic wrinkles by relaxing the muscles that cause them. It's most effective when used before dynamic wrinkles become permanently etched static wrinkles.
Botox by Age: What to Consider
In Your 20s
Should you start Botox? For most people in their 20s, the answer is: probably not yet.
At this age, your skin still has excellent collagen and elastin production. Dynamic wrinkles (lines when you move your face) are normal and don't require treatment. The focus should be on:
- Sun protection (SPF 30+ daily — the single most effective anti-ageing measure)
- Good skincare (retinol, vitamin C, hyaluronic acid)
- Healthy lifestyle (hydration, sleep, balanced diet)
Exception: If you have very strong facial muscles that create deep dynamic wrinkles even in your 20s (some people are genetically predisposed), a conservative approach with low-dose Botox may be appropriate.
In Your 30s
This is when most patients benefit from starting Botox. By your early to mid-30s, you may notice:
- Forehead lines that linger after you stop raising your eyebrows
- Frown lines that are faintly visible at rest
- Early crow's feet that don't fully smooth out
This is the ideal time for preventative Botox. By treating these areas before the lines become deeply etched, you can maintain a smoother, more youthful appearance for longer.
Typical treatment at SW11 Medical Clinic:
- 1 area (e.g., frown lines only): from £175
- 3 areas (forehead + frown + crow's feet): £320
- Conservative dosing for natural results
In Your 40s and Beyond
Botox remains highly effective in your 40s, 50s, and beyond. However, the approach may differ:
- Higher doses may be needed for stronger, more established wrinkle patterns
- Combination treatments (Botox + fillers + Profhilo) often produce the best results — learn more in our Profhilo vs Botox vs Fillers comparison
- Expectations should be realistic — Botox smooths wrinkles but doesn't replace lost volume
At this stage, we often recommend a comprehensive assessment to determine the best combination of treatments for your goals.
How Often Do You Need Botox?
Botox typically lasts 3–6 months. Most patients settle into a routine of treatments every 4–6 months. Interestingly, with regular treatment over time, many patients find they need less frequent sessions as the muscles gradually weaken.
The Importance of Choosing the Right Practitioner
Regardless of your age, the most critical factor is who administers your Botox. In the UK, Botox injections are not regulated as strictly as they should be — meaning non-medical practitioners can legally perform them.
At SW11 Medical Clinic, we believe Botox should only be administered by:
- GMC-registered doctors or
- NMC-registered nurse practitioners
Our team includes Dr Alessio Mocci (Harvard Medical School scholar, Harley Academy trained), Dr Maryna Startseva (14+ years aesthetic medicine experience), and Żaneta Zbierska (BACN-registered aesthetic nurse).
We use only genuine, FDA-approved botulinum toxin (Allergan Botox / Azzalure) in our CQC-registered clinical environment.
Signs You Might Be Ready for Botox
Consider booking a consultation if:
✓ You notice lines on your forehead or between your brows at rest
✓ Your crow's feet are becoming more prominent

Signs You Probably Don't Need Botox Yet
